Just Got Married? Here's How to Change Your Last Name with Tips from the Bride and Breakfast Facebook Community

Passport

Bride Em from our Facebook Community decided to update her passport first because this was the most convenient document to process. The requirements she prepared were: old passport and PSA marriage certificate.

I started with renewing my passport so that I have a government ID that’s already in my married name, old passport and PSA marriage certificate lang kasi need and mabilis lang nanjan na in a week. Everything else kasi would need a government ID na nakalagay na ang married name mo.” – Em Parac, Bride and Breakfast Facebook Community

Process

  • Set an appointment on DFA website
  • Fill up the application form for passport renewal using your married name
  • Pay for the processing fee within 24 hours
  • Head over to the DFA office on your scheduled appointment

Driver's License

For bride Meryem, updating her driver’s license first was the fastest way she can have her new identification card with her married name. The requirements she prepared were: old driver’s license and marriage certificate (original and photocopy).

The first ID that I was able to change was my driver’s license, all you need to bring is your original marriage certificate and one photocopy, and of course your license. It took me only 30 minutes and it is just 325 pesos.” – Meryem Tan, Bride and Breakfast Facebook Community

Process

  • Head over to the nearest LTO branch and submit all the requirements
  • Settle your payment and wait

SSS

One of the brides, Gen, shared that you can actually update your married name on SSS online. However, she also noted that you have the option to go directly to the branch if you want it to be processed right away. The requirements she prepared were: valid ID and marriage certificate.

You can file it online, upload lang ID and marriage contract. Medyo matagal lang ma-approve, inabot ako ng almost a month so ok if direct sa branch.” – Gen Young-Gili, Bride and Breakfast Facebook Community

Process

  • Head over to SSS website
  • Log in to your account and select “e-services” from the menu
  • Click “request for member data change”
  • Fill out the important fields and upload your valid ID and marriage certificate
  • You will receive an email from SSS about your request

Pag-IBIG

When your HR isn’t able to update your Pag-IBIG and other documents, bride Na-na told the other brides from our Facebook Community that they can go straight to the government services center at a mall near their place. The requirements you’ll need to submit are: valid ID and marriage certificate (original and photocopy).

Kung hindi ma-update ng HR nyo, go to SM government services center. One stop shop para mag update ng records. Pero para sa physical IDs, need pumunta sa branch.” – Na-na Guerrero, Bride and Breakfast Facebook Community

Process

  • Fill up the MCIF and head over to the nearest Pag-IBIG office
  • Submit all the requirements and wait until they update your information

PhilHealth

Bride Gen had a fast and smooth transaction with PhilHealth. The only thing is you have to go to the branch early to avoid long lines. Other than that, you will be able to receive your updated ID instantly. The requirements she prepared were: valid ID and marriage certificate.

“‘Yung PhilHealth sobrang bilis. Mahaba nga lang pila. Fill-up ka lang ng form with valid ID and marriage contract kahit yung galing sa local registry. Then pagsubmit mo sa kanila mag pi-print naman agad ng ID.” – Gen Young-Gili, Bride and Breakfast Facebook Community

Process

  • Fill out the PMRF and head over to the nearest PhilHealth branch
  • Submit the PMRF along with other requirements
  • Your status will be processed and you will be issued your new PhilHealth ID

Bank Records

Like the other documents, bride Em was able to update her bank records with no delays. She just brought with her the following: valid ID with married name and marriage certificate.

You have to go to the bank (depends on the bank if you need to go to branch where you opened your account or just any branch), again bring ID and marriage contract, done in a few minutes.” – Em Parac, Bride and Breakfast Facebook Community

Process

  • Head over to your bank (clarify with the bank if you have to go the branch where you opened your account, or if any branch will do)
  • Inform the teller you want to update your status and last name
  • Get an updated debit card
